LIABILITY WAIVER

In consideration of the risk of injury while participating in soccer training (the “Activity”) and as a condition for the right to participate, I, on behalf of myself and my child/participant, hereby knowingly and voluntarily enter into this Waiver and Release of Liability. I understand and acknowledge that participation in the Activity involves certain inherent risks, including but not limited to physical or psychological injury, illness, paralysis, disability (temporary or permanent), emotional or economic loss, property damage, or death. These risks may arise from my own actions or inactions, the actions or inactions of others, the conditions of the facility, weather, equipment, or travel to and from the Activity. By signing below, I voluntarily assume all such risks, known and unknown, and fully accept personal responsibility for any injury, loss, or damage that may occur during or as a result of participation in this Activity, including travel to and from any related events. I hereby release, discharge, and hold harmless F3 Soccer of USA, LLC (DBA F3 Soccer Academy), its staff, coaches, volunteers, affiliates, successors, and assigns from any and all claims, liabilities, or causes of action arising out of or in connection with my or my child’s participation in the Activity. This release includes, but is not limited to, claims for negligence to the fullest extent permitted by law. I agree to indemnify and hold harmless F3 Soccer of USA, LLC from any claims, suits, actions, or expenses (including attorney’s fees) arising out of my or my child’s participation in the Activity, including any damage caused by our willful actions, negligence, or recklessness. I understand that this Activity may test physical and mental limits and may involve risks such as: • Uneven terrain or facility hazards • Weather-related conditions • Hydration or temperature issues • Participant behavior and equipment use • Vehicular or pedestrian traffic near the facility In the event that medical care is required, I agree to be financially responsible for any associated costs and understand that it is my responsibility to carry personal health insurance. Should any damage occur to the facility or equipment due to the actions of myself or my child, I acknowledge financial responsibility for the cost of repairs or replacement. By signing below, I confirm that I have read, understood, and voluntarily agree to the terms of this Waiver and Release of Liability. I acknowledge that I am giving up certain legal rights, including the right to sue.
